How We Collect Your Information
We collect and keep information about you, which is needed to provide the products
and services you request. This information can include: financial records, names,
social insurance numbers, ages, dates of birth, sex, marital and family status,
occupations, salaries, employment histories, smoking status, claims experience,
home addresses and telephone numbers and medical backgrounds. This information is
collected directly by us. This information is sent directly to private labs,
insurance and investment companies who will underwrite or quote on your application.
We do not receive copies of your medical information. We will maintain an internal
database, as well as keep files to record personal information, underwriting follow
up, conversations that we may have concerning your application and future notes
pertaining to policy service. This information is located in a secure section of
our office and is accessible only by those directly involved with the services we
provide. We may also access information concerning your insurance policies,
investments and benefit plans directly with the Insurance and Investment Companies
to provide service to you or an authorized representative.
If your authorized representative requests information concerning the status of your insurance policies, investments or benefit plans, this information will be provided unless a subsequent authorized representative is appointed and we receive a written letter of authorization from you.
Your Right to Access Your Information
